Continuum of Care Committee
The Continuum of Care committee works to ensure collaboration across disciplines to support access to a full continuum of services/care for youth and families with mental health needs. It seeks to Increase community, educators and families understanding of the link between social emotional learning/mental health/trauma and academic achievement. The committee's current goal is to: (a) assist Collaborative to have common language/goals around advocacy, (b) develop a framework to understand our continuum of care to identify the gaps and get to the work of advocating real change, and (c) strengthen connections between systems to ultimately to build supports for families/consumers, and (d) create best practices across systems & enhance collaboration.
